How to Get Started: If you make a qualifying purchase after April 30, 2013 please download and complete the regular rebate form on new homes or existing homes,
complete the appropriate form and either fax the completed form and a
copy of the applicable receipt to 891.6127 or mail them both to:

Please note: The Double Rebate program ran from February 14, 2013 thorough April 30, 2013. Double Rebate applications must be received and all installations completed by June 14, 2013. If you made a qualifying purchase during that time, read the DOUBLE REBATE terms and conditions, and fill out the downloadable form for DOUBLE Rebates (new homes) of DOUBLE Rebates (existing homes) and either fax the completed form and a copy of the applicable receipt to 891.6127 or mail them both to:
