Minor or Major Modification Request for Site Plans and Subdivisions
Modification Authorization:
To determine whether the changes you are proposing will be considered a "minor" or a "major" modification, please contact the Growth Management Department, Land Use and Environmental Services Review Team (850-891-7100, option 4) for the area of town your project is located in. They will give you a letter authorizing your submittal as a "minor" or a "major". This letter must be uploaded with your modification application into City Projects.
Application Fees
After your application has been applied for and reviewed by intake staff, you will receive an e-mail providing you with a link to make your payment online.
- Minor modification - $525.00
- Major modification - $698.00
*** Type B/Preliminary Plat/Limited Partition MAJOR modifications also require a $475.00 legal notice fee and are subject to all notice requirements.

- Modification Determination Letter
- Modification Narrative
- Boundary Survey
- Site Plan with Changes (PDF)
- All documents should also be submitted in electronic PDF form using the Standard Naming Conventions (PDF)
